The Hamilton Niagara Haldimand Brant Community Care Access Centre (HNHB CCAC) connects people in our region with the health and support services they need to: remain at home, avoid hospital admission, access support upon discharge from hospital and explore long-term care options. At any time, you can call back to ask more questions, or to enquire about other services. You can receive a customized "My Health GPS" information package by email or Canada Post describing the services you select, contact information, hours of operation, including location, eligibility criteria and more. This means they have the skills and experience to understand your unique situation, working with you to identify the services that may help you. Your call will be answered by a highly trained CCAC agent, who is a certified information and referral specialist. It starts with a free phone call to 1-800-810-0000.
